ermha360 is a community mental health and disability support service operating in the Barwon region. ermha360 runs the Pathways to Community program in conjunction with Community Housing Limited. ermha360 will provide support to you as a Pathways to Community resident. Pathways to Community will support you to build a stable rental history, develop life skills and achieve greater independence over time. For some the journey may be as short as 6 months, for others it may require more time. In any case you can build your place in the community from a secure home with the right support.

Description

This development consists of three independent units at the end of a quiet street.
They provide a home for young people with a disability. Also on site is one unit for the
support provider which features an office with therapy space for residents and a
bedroom for on-site overnight assistance.
CHL is responsible for the on-going property management while ermha360
provides 24 hours support.

All buildings have been designed by CHL’s in-house architects and comply with current disability design requirements under Australian Standards (AS) and the National Disability Insurance Scheme (NDIS).

The following are the eligibility criteria for occupation of the property:

• Participation in the NDIS

• Be over 18 years of age

• Have the desire to live independently

Ideally, the tenant’s NDIS plan includes eligibility for SDA at a ‘Basic’ level.
Where no future tenant can be identified that is eligible for SDA at a ‘Basic’ level, the future tenant could also be selected on the basis that he/she requires the building design features recommended for ‘Basic’. An application for SDA eligibility would be submitted when moving in.
